Initial Women’s Sexual Health Visit

Consultation

Before your sexual health consultation visit in Paducah, you will receive some forms through the patient portal. These should be filled out prior to the visit so that the provider will have a clear idea of the problems you would like to discuss. During the visit you will discuss your concerns and symptoms. Examination and laboratory testing may be done. You should allow 60-75 minutes for your first visit. Within 48 hours, you will receive a digital summary of the visit with education material through our app.

Follow Up Visit #1

At your first scheduled follow up visit, lab results, diagnosis, and treatment options will be discussed. Together, we will determine the best combination of treatments based on your needs and goals which may include medications, education, counseling, or therapy.

Follow Up Visit #2

At your second follow up visit, we will discuss your improvement and progress. We will adjust treatments and/or dosage of medications if needed. We will determine the next steps based on your current progress and goals.

Fees

    • Initial Evaluation all new patients (60-75 minutes) $275

      Includes thorough intake, a customized wellness plan with your concerns and goals at the forefront, lab ordering and review if appropriate, and solution focused treatment recommendations. After your visit, you will receive a comprehensive, individualized wellness plan in PDF format with relevant education materials.

      Follow-Up Counseling (50 minutes) $150

      Sexual Health and/or Hormone Follow Up (30 minutes) $100

Frazine Wellness does not currently participate with commercial insurance plans and is considered an out-of-network provider. Patients benefit from this model by experiencing longer appointments and more personalized care. You can use your insurance for labs and possibly for medications.

At the end of your appointment, you will be provided with an itemized payment summary. Upon request, you may request a superbill for you to submit to your insurance for possible out-of-network reimbursement. Our office accepts payment forms of: Health Savings Account (HSA), Flexible Spending Account (FSA), Credit Card, Cash, Check, Care Credit, or Cherry.
